Apply to Become a Provider →All requirements must be met before your account can be activated. Most providers are approved within 48 hours.
You must be legally authorized to work in the United States and reside in or near the Maryland service area (Baltimore, Columbia, Rockville, and surrounding counties).
RequiredAll providers must be at least 18 years of age. A valid government-issued photo ID (driver's license, passport, or state ID) is required during registration.
RequiredA comprehensive 5-layer background check is conducted including criminal history, sex offender registry, identity verification, and Maryland county records.
RequiredAt least 1 year of caregiving experience OR a valid certification (CNA, HHA, PCA, CPR/First Aid). Providers with more credentials earn higher match scores.
RequiredRequired only for providers offering transportation services. A clean MVR (Motor Vehicle Record) with no major violations in the last 3 years is required.
Transport OnlyProviders are strongly encouraged to maintain personal liability insurance. Auto insurance at Maryland minimums is required for transport-eligible providers.
RecommendedAn Android smartphone is required to run the Prezivio Provider App. The app uses GPS for real-time tracking, mileage calculation, and service verification.
RequiredA valid U.S. bank account (checking or savings) is required to receive earnings. Payments are deposited directly on a rolling basis after service completion.
RequiredRN, LPN, CNA, BLS, Alzheimer's/Dementia training, Med Pass, or Hospice certifications significantly boost your AI match score and visibility to seniors.
